Privacy policy.
01 Scope
This policy describes how information submitted through tampapractice.com (the "site") is collected, used and safeguarded. The site is operated by or on behalf of Dr Pid'ange Tampa-Sahum, a Family Physician practising at Peel Family Medical Centre, Brampton, Ontario.
This policy applies to the public website only. Clinical care provided at Peel Family Medical Centre is governed by the clinic's own privacy practices and by Ontario's Personal Health Information Protection Act, 2004 (PHIPA). It does not cover any third-party service you reach by clicking a link from this site — those services have their own privacy policies.
02 What we collect
Through this site, we may collect:
- Information you choose to submit through the contact form (name, email address, nature of enquiry and the content of your message).
- Information you submit when purchasing an educational product from the store (name, billing address, shipping address for physical items, and payment-related tokens handled by our payment processor).
- Server-level technical information generated automatically when any website is accessed — your IP address, browser type and version, device information, timestamps and the pages you visited — used for security and performance.
We do not ask for and should not receive personal health information through this site. If you include such information in a form or email, see section 4.
03 How we use it
Information you provide is used only to:
- Respond to your enquiry.
- Fulfil any order you place from the store (process payment, deliver digital products, ship physical items).
- Comply with legal, regulatory or professional obligations.
- Maintain the security and performance of the site.
We do not sell personal information to anyone, and we do not use it for advertising to you or anyone else.
04 Health information & PHIPA
If you are a patient of Dr Tampa-Sahum and need to communicate about clinical matters, please contact Peel Family Medical Centre directly on 905 451 5586 or use the booking system at app.avaros.ca/av/weblink/peel.
If health information is submitted through this site despite the above, it will be treated with the confidentiality required by PHIPA, deleted from this site's systems as soon as reasonably possible, and not retained unless retention is required by law or professional regulation.
05 Third-party services
The site relies on a small number of third parties to function. Each has its own privacy policy:
- Avaros — the clinic's booking platform. Clicking a "Book" button opens the Avaros website in a new tab; any information you enter there is handled by Avaros and the clinic, not this site.
- The site's hosting provider — serves the pages and may retain standard server access logs for security and performance purposes.
- Our payment processor (for store purchases) — handles your payment card details directly. We receive only confirmation of payment and the information needed to fulfil your order.
- Shipping carriers — if you order a physical product, your name and shipping address are passed to the carrier to deliver it.
06 Retention
Contact-form messages are retained for as long as reasonably needed to respond and typically no longer than twelve months. Order records are retained for seven years to comply with Canadian tax and accounting rules. Server access logs are retained for a short period only, consistent with security best practice.
07 Cookies & tracking
At the date of this policy, the site uses no analytics cookies, advertising cookies or tracking pixels. If this changes — for example, if a privacy-respecting analytics tool is added — this page will be updated and a cookie-consent notice will be shown before any such cookie is set.
08 Your rights
If you have submitted personal information through this site, you have the right to request access to it, correction of inaccuracies, and (where applicable) deletion. If PHIPA applies — because the information concerns your health and is held in connection with the provision of care — your rights under that Act apply directly.
To exercise these rights, contact us using the details in section 10.
09 Changes
This policy may be updated to reflect changes in the site, in applicable law, or in the services used to operate it. The "last updated" date at the top of this page always reflects the current version. Material changes will be flagged on the site.
10 Contact
For questions about this policy or how your information is handled, please use the contact form and select "General question" or email the clinic directly.
For complaints you cannot resolve through us, the regulator for health-information privacy in Ontario is the Information and Privacy Commissioner of Ontario (ipc.on.ca).
